Login at [https://app.graphiumemr.com](https://app.graphiumemr.com) 2. Confirm the correct facility is indicated in the upper left dropdown menu. ![Provider page-1](https://help.graphiumhealth.com/hubfs/Provider%20page-1.png) 5. If the provider does not already exist, click the red "+ New Provider" button in the top right. ![](https://help.graphiumhealth.com/hubfs/image-png-Jul-19-2022-04-08-09-15-PM.png) 7. Fill in the fields with the new provider's info. **Note: please ignore "LOCAL PROVIDER ID".** ![](https://help.graphiumhealth.com/hubfs/image-png-Nov-15-2022-05-27-33-8312-PM.png) 8. Once you have entered the provider's information, click the red "Create" button in the bottom right hand corner.  ![](https://help.graphiumhealth.com/hubfs/image-png-Jul-19-2022-04-13-39-70-PM.png) 9. If you need to deactivate a provider who has left your facility, simply search for the provider's name in the search box.  11.
